Patients without RAMQ or patients who use a private health insurance or without medical insurance, or international students have to submit this form.
Insurance for international students:
Our clinic is also affiliated with Desjardins and Blue Cross International Student Insurance and is approved by this insurance. Therefore, students must present their valid insurance certificate and student card when taking an appointment, and they do not need to pay more.
